Medical Release Forms
Players must provide a signed Medical Release Form for each season of Competitive Soccer. Please use the following instructions to find and print your player's Medical Release Form.

1) After registering, login to your GotSoccer player or family account here.
2) Under the player's Currently Enrolled Programs, click on "Details"
3) Under Additional Forms, click on "Printable Registration Form" to access and print your player's Medical Release Form.
4) Once you have signed the Medical Release Form, please give it to your coach or Age-Level Coordinator at tryouts or the first day of practice (if there are no tryouts). No player may participate without a completed and signed form. If you have not previously provided a birth certificate, please email a copy to registrar@pysa.com
or upload it securely to your player's GotSoccer player account. Also, please ensure that a recent passport style photo has been uploaded to your child's GotSoccer account. Your child will not be placed on a team roster without both an appropriate photo and verified birth certificate.
